OR WCD Revises Spanish Forms

The Oregon Workers' Compensation Division (WCD) has published Bulletin 310, "Spanish language Forms 440-801S, "Reporte de Lesion o Enfermedad en el Trabajo" (Report of Job Injury or Illness") and 440-3283S, "Entendiendo las Reclamaciones de Compensacion para Trabajadores" (Understanding workers' compensation claims")". The purpose of the bulletin is to announce the availability of revised Forms 801S and 3283S. This bulletin supersedes Bulletin 310 dated May 17, 2002.
