OR Form 3530 Amended

Oregon Workers' Compensation Division Bulletin No. 325 regarding supplemental disability payment and reimbursement was revised effective January 14, 2005. The purpose of this bulletin is to provide forms and formats for supplemental disability processing and reimbursement under OAR 436-060-0035 and 0500 and an amended Form 3530, "Supplemental Disability Election Notification." Form 3530 has been amended to eliminate the year designation with regards to how long the assigned processing agent is elected to process supplemental disability claims.
