Comp Commission Announces Changes to Death Claim Notice Form

The South Carolina Workers’ Compensation Commission on Wednesday announced that it edited language on the employee death claim notice for the purposes of clarity and judicial expediency. The commission said the revised Form 52 — “Employee’s Notice of Claim and/or Request for Hearing, Death Case” — added fields for the decedent’s name and Social Security number, as well as the claimant’s name and Social Security number.
