The Virginia Occupational Safety and Health Program issued $84,000 in penalties against Georgia Pacific in Big Island for safety violations discovered after a worker fell through a hole in the floor and drowned in a pulp tank.
VOSH inspectors determined that the company had removed a nip roll guard from a floor opening, OSHA said in an account published in its Quick Takes newsletter. VOSH’s citation states that Georgia Pacific removed the guard on Nov. 16, 2016. The employee fell through the unguarded hole in the floor on Nov. 23, 2016.
