Facts: Gregory Moore injured his back while replacing a mining belt in October 2012. He sought treatment in Williamson Memorial Hospital's emergency department and reported back pain that radiated into his left leg.
Radiographs of his back revealed mild degenerative changes, and doctors diagnosed him with lumbago and sciatica.
