The state agency that oversees self-insurance programs released new application forms on Wednesday. The Department of Industrial Relations' Office of Self-Insurance Plans issued new copies of the following private stand-alone application forms: Form A-1: Application for a Private Entity Certificate of Consent to Self-Insure.
