New Form Developed to Determine Second-Injury Fund Eligibility

The Louisiana Office of Workers’ Compensation has proposed a rule regarding a questionnaire given to newly hired workers or those who have received a conditional job offer. The form asks workers about medical conditions and past injuries. It is used to help determine whether an employer is eligible for reimbursement from the Louisiana Workers’ Compensation Second Injury Board if hurt on the job.
