Agency Revises Request for Transcript Form

The Maryland Workers’ Compensation Commission has revised its H50 form, Request for Transcript, so that stakeholders can complete the form in a PDF format. The revised form includes additional instructions about requesting a transcript from the agency’s Court Reporting Division. Transcripts requested for appeals will be available within 60 days after the agency receives the first petition for judicial review.
