Facts: Jerome Matthews worked for HydroChemPSC. In July 2018, his employer sent him to the Phillips 66 Company Alliance Refinery in Belle Chasse, Louisiana. According to local news reports, Matthews fell into a water basin in the refinery’s primary cooling tower and drowned.
