VOSH Cites HVAC Company After Worker's Fatal Fall

Virginia Occupational Safety and Health issued $225,995 in penalties to James River Air Conditioning Co. in Richmond after a worker fell off a roof and died.  The worker fell 28 feet through an unmarked, unsecured hole cover that did not hold his weight.  The company was issued six citations.
