DWC Revises Death Benefits Forms

The Texas Division of Workers’ Compensation announced revised forms it said will help eligible beneficiaries file claims for death benefits. The division updated DWC Form-042, “Claim for Workers’ Compensation Death Benefits,” and PLN-12, “Notice of Potential Entitlement to Workers’ Compensation Death Benefits.” DWC said the form revisions are necessary to implement Texas Labor Code Sections 408.182 and 409.007, which were amended by House Bill 2314 this year.
