Regulators Post Draft of Medical Release Authorization Form

The Department of Labor has posted a revised draft of Form 7, which is required of all providers when seeking or providing medical information related to a workers’ compensation claim. Title 21 VSA §655a states the agency must approve a form for use by health care providers who examine or attend the examination of an injured worker.
