The Occupational Safety and Health Administration said three investigations into the April death of a worker at an Oklahoma manufacturing plant resulted in a total of $275,890 for 36 alleged violations.
OSHA opened an initial investigation at Accurate Manufacturing Inc. in Tulsa after the fatal accident. The agency then opened comprehensive safety and health investigations under an active Regional Emphasis Program focused on hazards common in the fabricated metal products industry.
