WCC Revises Form, Clarifies 'Set-With' Requirements

The Maryland Workers’ Compensation Commission has revised form H25R, Request for Action on Filed Issues. The revised form clarifies required actions and items for “set-withs,” and includes the option for “health care provider” as a filing party. “Set-with” is used when the filing party has more than one workers’ comp case to be heard.
