DWC Seeks Comments on Changes to Death Benefit Forms

The Texas Division of Workers’ Compensation is accepting public comments on changes to DWC Form-042, “Claim for Workers' Compensation Death Benefits,” and PLN-12, “Notice of Potential Entitlement to Workers' Compensation Death Benefits.” The changes implement Texas Labor Code Sections 408.182 and 409.007, which were amended by House Bill 2314 to help eligible beneficiaries file claims for death benefits with the DWC or an insurance carrier.
