A Temporary Exception to the New RU Forms

Use of the revised RU forms became mandatory May 1, 2003. Forms affected are the RU-94, RU-102, RU-103, RU-105, RU-120, and RU 122 as well as the RB-105. The RU-101 has been eliminated; the information necessary for the Unit to set up a file is included on each action document. The Unit will reject filings where the employer/insurer has used old versions of the RU forms identified above.

There is one important (but temporary) exception to the Unit's requirement to use the new forms exclusively. The Unit will accept rehabilitation plans where the RU-102 was developed, and the signature process commenced, prior to May 1, 2003. This means the QRR signed the plan prior to May 1, 2003 and sent the document to the applicant's attorney for review and signature. In such cases, Otis Byrd recommends that the carrier/employer file the RU-102 with the Unit with a cover letter explaining that the plan was developed and signed before use of the new forms became mandatory.
