The Texas Division of Workers’ Compensation announced that two revised benefit forms were updated and are available for immediate use.
The DWC in June 2021 announced a plan to update its notices and forms with a new letterhead and mailing address. The plan included updates to the English and Spanish versions of some forms.
The division said two forms are ready for immediate use:
DWC Form-031, “Application for Division Approval of Change in the Payment Period and/or Purchase of an Annuity for Death Benefits.”
