A formulary would work ONLY if the DWC is responsible enough to establish rules for both the treating physicians AND the insurance carriers. Do not forget that UR has stymied physician prescriptions time and time again with excessive UR delays. I still marvel at the concept of denial for NSAIDS (cheap & effective) by UR, then IMR. If the DWC can remain conscious long enough to ensure quicker response and approval of the simplest of common drugs, then a formulary might work.
