DIR Publishes Revised Forms

The Nevada Division of Industrial Relations announced that it revised some workers’ compensation forms earlier in October that must be used immediately.

The division posted the following revised forms online:

• D-1 “Informational Poster.”
• D-2 “Brief Description of Your Rights and Benefits if You Are Injured on the Job.”
• D-35 “Request for a Rotating Physician or Chiropractor.”

The forms were revised Oct. 20. The division said use of the forms became mandatory Thursday.
