Amputation Prompts $151,329 Fine

The Occupational Safety and Health Administration fined a New Jersey manufacturer $151,329 for safety violations after a worker suffered an amputation in a workplace accident.

A worker was cleaning a machine for BWAY Corp., operating as Mauser Packaging Solutions, in September 2019 when the amputation occurred.

OSHA cited BWAY Corp. for two repeated violations and two serious violations of failing to follow lockout/tagout procedures. The federal workplace safety agency said it cited the company for similar violations in 2016 and 2019.

The company has 15 business days from receipt of the citations to comply, request an informal conference with OSHA's area director or contest the findings before the Occupational Safety and Health Review Commission.
