The Oregon Workers’ Compensation Division is holding a hearing Nov. 16 on a proposal to modify claim forms to remove fields used to report the Social Security numbers of injured workers.
The division said it is proposing to remove the Social Security number field on Form 801, “Report of Job Injury or Illness.”
