Renewal Forms and Guidelines for Self Insurers Available

The Indiana Workers’ Compensation Board has released the 2015 guidelines and forms for self-insurers, which are due by July 31.  The forms, which can be found here, will not be processed until the application is finished in full and a renewal fee of $250 is attached, the board said in an announcement. There is also a $250 late fee implemented if filers do not meet the July 31 deadline.  The memo on the board's website noted that the application should be completed if the company is revising any subsidiaries.
