Refinery Hit with $324,000 Penalty for Uncorrected Safety Violations

The Washington state Department of Labor & Industries fined Phillips 66 Refinery in Ferndale $324,000 for failing to correct serious workplace safety violations identified during a 2014 inspection. L&I inspections in September and October of 2014 led to citations for not following generally accepted engineering practices with regard to a firefighting water tank and a buried firefighting water distribution piping system.  Phillips 66 is challenging the citations, L&I said.
