Glass Manufacturer Fined $77,000 for Amputation

The federal Occupational Safety and Health Administration fined a Georgia glass manufacturer $77,000 for one willful and one serious safety violation after a worker suffered a finger amputation while removing a glass mold from a bottle-shaping machine.Ardagh Glass Inc. failed to develop and use lockout/tagout procedures when workers are performing service and startup operations on machines, OSHA said.
