Industrial Commission Posts Revised Record Request Forms

The Idaho Industrial Commission posted to its website revised forms that should be used to request workers' compensation claims records starting next year. As part of the process, the commission created a new specialized records request form, RMR-6, to be used when requesting records on claims closed before Jan. 1, 2004, that are maintained on microfilm. Other revised forms include RMR-1, used by parties to an open or closed claim to request records, and RMR-4, the employee's release form permitting non-parties to receive claim information.
