All third-party administrators in Nevada are required to file fiscal year 2014 information forms with the Workers' Compensation Section of the Division of Industrial Relations by Nov. 26.
Information requested on the forms includes physician and mailing addresses, contact numbers, license numbers and the company's Federal Employer Identification Number.
TPAs are also required to report the total number of claims handled during the fiscal year that ran from July 1, 2013, through June 30, including the number of accepted, denied and reopened cases.
