An Illinois bread manufacturer's failure to power down machines during cleaning led to two workers suffering severe injuries in a six-week period, Occupational Safety and Health Administration inspectors found.
A 52-year-old worker cleaning a bread crumb conveyor in August lost his hand when he placed it into the machine. Six weeks later, a 57-year-old man fractured his forearm in multiple places when a mixing blade rotated unexpectedly while he was cleaning a dough machine.
