Texas sheet metal maker M&M Manufacturing has been cited by safety inspectors for failing to protect workers from amputation hazards by failing to install guards on its machines.
The company has previously been cited by the Occupational Safety and Health Administration for similar lack of guarding at three different facilities between 2012 and 2015.
OSHA proposed penalties of $70,000 following the Sept. 30 inspection, initiated after the agency received a complaint alleging an amputation injury.
