A Jacksonville, Florida, roofing company faces $154,000 in penalties for not protecting one of its workers who crashed through a skylight, according to the Occupational Safety and Health Administration.
John W. Miles III died when he fell over 24 feet after falling through the skylight, which could have been avoided had his employer, Pinnacle Roofing Contractors Inc., installed safety cages around the skylight, the agency said in a press release.
