Safety violations that included tangled extension cords, overloaded power outlets in wet locations, blocked exits, faulty forklifts, machines without safety guards and damaged floors prompted the U.S. Occupational Safety and Health Administration to propose $63,000 in fines against a Jersey City warehouse owner.
