Fatal Trench Collapse Earns Firm Spot in Severe Violator Program

A Pennsylvania bridge repair and construction company earned $140,000 in proposed penalties and a spot on the U.S. Occupational Safety and Health Administrator's severe violator list after a trench collapse killed an employee. OSHA said it issued two willful citations to Susquehanna Supply Co. of Williamsport after inspecting the site of the accident near Little Fishing Creek on State Route 4201 in Millville, Pennsylvania.
