A Pennsylvania bridge repair and construction company earned $140,000 in proposed penalties and a spot on the U.S. Occupational Safety and Health Administrator's severe violator list after a trench collapse killed an employee.
OSHA said it issued two willful citations to Susquehanna Supply Co. of Williamsport after inspecting the site of the accident near Little Fishing Creek on State Route 4201 in Millville, Pennsylvania. Investigators determined that the employee who died was working inside a trench that was 12 to 15 feet deep when it collapsed on him and he became buried in soil.
