Employer Faces 28 Citations, $77K in Fines after Death

The Occupational Safety and Health Administration cited Phoenix Industrial Cleaning for 28 serious safety violations after a worker died while working inside of a storage tank. The federal agency investigated the industrial cleaning business after an employee was overcome by methylene chloride vapors while working inside a tank at a chemical-manufacturing facility in Wheeling, Ill.
