California workplace safety regulators fined Quantum Energy Storage Corp. $58,025 for a failure to follow safe machine operating standards that resulted in an explosion, injuring four workers.
Cal-OSHA said a 7-foot, 11,000-pound metal flywheel used to test the energy storage system at Quantum's warehouse in Poway came loose from its moorings on June 10. The flywheel, which was spinning at 6,000 rotations per minute, destroyed the concrete vault that housed it and caused an explosion so destructive that the warehouse was deemed unsafe to enter.
