Forms Must Note Exact Nature of Injury, Board Says

Starting Monday, applications for adjustment of claim that do not specify the nature of the injury will be considered incomplete and will have to be amended, the Indiana Workers' Compensation Board said in a bulletin. SF 29109 forms that are submitted with only vague wording, such as “plaintiff sustained a work-related injury in the course and scope of employment,” will have to be fixed by sending in a new and more complete application, says the bulletin, posted Wednesday.
