DWC Corrects Wage Statement Forms

The Texas Division of Workers’ Compensation corrected three recently adopted forms relating to wage statements to add bar codes and remove contact information from the letterhead. The division on Friday said it corrected DWC Form-003, “Employer’s Wage Statement,” DWC Form-003ME, “Employee’s Multiple Employment Wage Statement" and DWC Form-003SD, “Employer’s Wage Statement for School Districts."
