OSHA has cited food plastics manufacturer Liqui-Box Corp. in Ashland for three serious safety violations for exposing workers to machine safety hazards.
The agency has proposed a fine of $42,413.
Liqui-Box failed to report to OSHA within 24 hours a worker’s hospitalization after he suffered a partial amputation of his left thumb when a molding press slammed shut on his hand.
OSHA inspectors said the company also did not adequately guard machine operating parts, implement lockout/tag out procedures and train workers on those procedures.
