WCD Changes Retroactive Program Reimbursement Form

The Oregon Workers’ Compensation Division updated the form used to request payments from the retroactive benefit program. The WCD said changes to Form 3285 replace references to “invalid” children with references to “incapacitated” children; substitutes “surviving spouse” for “widow;” and updates the division’s contact information.
