An advisory rulemaking committee will meet Oct. 15 to discuss removing fields for Social Security numbers from claim forms, the Oregon Workers’ Compensation Division announced.
The division said it recently met with system users and determined to proceed with plans to eliminate the requirement to provide a Social Security number on Form 801, “Report of Job Injury or Illness,” and Form 827, “Worker’s and Physician’s Report for Workers’ Compensation Claims.”
