The Occupational Safety and Health Administration has proposed $55,000 in fines for Lone Star Management in Pittsburg, Kansas, after one worker died and another suffered serious burns in a fire in August.
A supervisor directed the two workers to use a gas-powered forklift to move pallets of fireworks out of a storage facility on Aug. 12, according to a press release. The gas from the forklift ignited, leading to an explosion and subsequent fire. The two employees were trapped. A 28-year-old worker died, while a 43-year-old suffered burns on 80% of his body.
