The California Division of Occupational Safety and Health is investigating how three workers were burned by hot metal Wednesday at Bay Area Tesla Motors, Inc. in Fremont.Tesla has been cited twice for safety violations since 2011, according to CalOSH records. It was fined $5,400 in 2011 and $3,450 in 2012 for serious violations at its Fremont assembly plant.
