WCD Posts Revised Forms

The Oregon Workers’ Compensation Division issued bulletins announcing the adoption of forms needed for processing claims and reporting medical data. The WCD in Bulletin No. 101 announced revisions to Form 3283, “A Guide for Workers Recently Hurt on the Job,” that include updated email addresses and an updated division logo.
