The Nevada Division of Industrial Relations promulgated revised forms for reporting injuries and occupational diseases, and a third form describing the rights and benefits offered to people hurt on the job.
The DIR updated the C-1 Notice of Injury or Occupational Disease form as well as the C-3 Employer’s Report of Industrial Injury or Occupational Disease form.
The division also released a revised version of the D-2 form describing the rights of injured workers.
Revised forms took effect Jan. 24, the DIR said in a statement issued Tuesday.
