The Occupational Safety and Health Administration fined an Ohio plumbing contractor $145,800 for allegedly exposing workers to trenching and excavation hazards.
The agency commonly referred to as OSHA said an employee for Payne Enterprises Inc. in Dayton was killed in a trench collapse at a residential construction site.
OSHA cited Payne Enterprises for two repeated violations for not having a competent individual inspect the trench before allowing employees to enter and failing to install an adequate protective system to prevent the trench collapse.
