The Utah Labor Commission replaced Form 219, "Permanent Partial Disability Compensation Agreement" with a new Form 219, "Statement of Compensation," that must now be sent to the injured worker along with the first payment of partial disability benefits.
The commission said its Industrial Accidents Division will no longer sign off on the old form.
Old Form 219 included a section dealing with attorney fees that has not been included in the updated form. Going forward, the Adjudication Division will manage attorney fees with a new Form 800, "Attorney Fee Approval" ...
