The California Division of Workers’ Compensation said lien claimants must now use a uniform assigned name when filing certain forms.The division on Friday afternoon said any lien claimant must use the UAN to electronically file a notice and request for allowance of a lien or an application for adjudication beginning Saturday, June 29.
