The Occupational Safety and Health Administration cited a Hawaii company specializing in rental and repairs of heavy equipment for 15 violations following an investigation into the 2012 death of a worker.An employee of Hawthorne Pacific Corp. in Maui was fatally crushed by a crane on Oct. 23, 2012, according to OSHA.
