DWC Seeks Comments on Proposed Form Revisions

The Texas Division of Workers’ Compensation is asking the public for input on proposed revisions to five workers’ comp forms, the agency announced. The proposed changes would cover: DWC Form-081, an agreement between a general contractor and subcontractor to provide workers' comp insurance.
