Forms Accepted Only Through Email, DIA Says

Starting July 1, the Massachusetts Department of Industrial Accidents will no longer accept certain forms through the mail, the department announced. Form 105, an agreement to extend the 180-day payment period, and Form 113, agreement to pay compensation, must now be filed via email. The forms can be sent to DIA-Form105conciliation@mass.gov and to DIA-Form113conciliation@mass.gov. Forms submitted by mail will not be reviewed, the department said. The DIA board number should be included in the upper right corner of the forms. A cover sheet is not needed. ...
