WCD Revises Forms to Reflect State Law Changes

The Oregon Workers’ Compensation Division issued two bulletins Tuesday, one dealing with the form and format for a worker’s request to change physician, and another related to the forms required for processing occupational disease or injury claims. Bulletin No. 251 revises Form 2332, "Request to Change Attending Physician or Authorized Nurse Practitioner."
