The U.S. Occupational Safety and Health Administration has cited a Jacksonville, Fla., welded wire manufacturer following the death of a 32-year-old worker who was struck and killed by a wire mesh machine in August 2013.OSHA said in a press release the worker was killed when he reached for a fallen metal bar because a light curtain that would have automatically turned off the machine at Wire Mesh Sales had been disabled.OSHA cited the company for eight willful violations and a series of other repeat and serious safety violations and has proposed fines of $697,700.OSHA put the company into its ...
